USA version 1.00. Made with DolphinHawk. Syncs with Dolphin 5.0-16793, and apparently also with 5.0-21264, according to aleckermit.

NOTE: It requires the following action replay code to sync: Unlock all levels: 00000000 8423F984 00000001 000C0001

Unfortunately, my SSD failed while I was working on the subtitles, so I'm waiting for a replacement drive to show up in the mail. (I didn't lose any data)
